Introduction

I qualified as an Occupational Therapist in 2008. Throughout my career I have developed the clinical knowledge, confidence and expertise to manage individuals presenting with complex conditions. My expertise in complex orthopaedic injuries, brain injury, dementia care and mental health conditions, enables me to assist individuals to return to their optimum function, whilst managing associated risks and expectations. I have worked as a case manager since 2017, which involves carrying out comprehensive assessments of individuals needs following a catastrophic event. My role enables me to work closely with the legal team, individuals, education and college settings, employers and families. I devise comprehensive reports outlining individuals’ strengths and limitations and make recommendations for appropriate rehabilitation. Part of my role involves working closely with the NHS and social care provision, setting up and monitoring care packages and facilitating multi-disciplinary meetings with clinicians.

As an occupational therapist and case manager, I am responsible for conducting functional assessments in order to make recommendations for the provision of treatment, which may entail major adaptations, equipment or rehabilitation. I have been responsible for, carrying out bio-psycho-social and risk assessments, contributing and implementing care provision, formulating treatment plans, collaborative goal setting, monitoring and evaluating treatment plans. I am able to communicate effectively and confidently with all parties involved on individual needs.

I have worked in discharge teams and social services, with individuals presenting with complex injuries and situations. I have also attended Best Interest meetings, completed care plans and risk assessments to ensure the best outcome is achieved.

I have gained sound knowledge of current legislation underpinning my practice, which includes, Mental Health Act, Safeguarding adults and children, College of Occupational Therapy code of conduct, Deprivation of liberty and the Mental Capacity Act. I have also developed a tool box of resources and contacts to support individuals’ progression in rehabilitation.

I accept instructions from both claimant and defendant solicitors and insurers. I have undergone formal training in the medico-legal process and am fully aware of my responsibilities under Civil Procedure Rules (CPR) pertaining to expert witnesses.
