Nurse practitioner, hospital avoidance, frailty, care at home
I registered as an general adult nurse in September 2011, and have worked within older persons’ medicine, emergency medicine and gastroenterology during the COVID-19 pandemic. I started my career within secondary care NHS organisations, holding roles as a staff nurse, junior nurse practitioner, ward manager and matron. I have gained considerable expertise; my specialised background has focused on elderly care, providing me with the skills and knowledge necessary to deliver compassionate, high-quality care to older adults, as well as assessing complex care needs and risks in relation to care provided. This experience has been both hospital-based and primary care-based. My current role as a nurse practitioner, focussing on work in care home settings by supporting the GP practices to prevent hospital admissions and ensure health at home, has given me up-to-date knowledge around risk assessments, safeguarding, mental capacity, and falls. Supporting a complex patient group and at times vulnerable establishments with complaints, CQC inspections, and commissioning monitoring.
Within my current role I assess, diagnose, treat and plan care for a caseload of patients. This aims to prevent hospital admission by providing an advanced nursing assessment to keep patients at home safely. I treat a number of medical conditions including chronic obstructive pulmonary disease, diabetes, hypertension management, heart failure, wound and rash assessment and long-term planning in relation to palliative and end of life care and concerns including acute infection of numerous sources, sepsis identification and management, identification of pressure ulcer development and nutritional status monitoring. I regularly as part of this role assess blood results and clinical tests relating to infections to ensure treatment is commenced swiftly for my patient caseload. I have held my non-medical prescribing qualification since 2023 and as part of this role, I regularly prescribe acute and long-term medication including the use and monitoring of controlled high-risk drugs and palliative care medication to promote patient safety.
Working as a ward manager and a matron, I have gained significant experience in report writing, particularly for coroners. As part of my responsibilities, I have been involved in preparing detailed and accurate reports for coroner investigations. These reports are crucial in providing a clear account of the medical care provided, the clinical decisions made, and the circumstances surrounding patient deaths. My ability to compile thorough, well-structured, and precise reports has been invaluable in investigations, ensuring transparency and accountability within the healthcare system. This aspect of my work has sharpened my attention to detail and my ability to communicate complex medical information clearly and professionally including presenting evidence and undertaking questioning in coroners court. My work in report writing demonstrates my commitment to upholding professional standards as I am passionate about delivering high quality care and ensuring care is fully documented and communicated correctly.
