I am currently employed as a clinical nurse lead with an Integrated Community Response Team based in West London. Patients range in age from 18 years to anyone who is fortunate to live beyond 100 years. The team adopts a full multidisciplinary approach. In some cases the team also provides a service for patients with mental health needs and learning disabilities. We aim to undertake an assessment within two hours of receipt of the referral following a crisis and need for health intervention or due to a breakdown of care.
I assess, plan, implement and evaluate care for patients with complex needs. This involves interpreting and analysing clinical and non-clinical facts to form accurate diagnoses and
prognoses and then recommending the best course of intervention. The team covers the A&E observation ward and acute assessment unit (AAU). Patients are referred to Accident and Emergency or be admitted directly to hospital within 24 hours after the initial assessment if required.
Being an Independent Prescriber within the team has really enhanced the nursing role and clarified its importance. When receiving referrals from GPs, community health practitioners,The London Ambulance Service and social care practitioners I can work efficiently as a sole practitioner - not having to rely on Doctors to prescribe basic medication thus freeing them up for medical assessments.
One of my goals has been to help reduce poly-pharmacy within our trust and to assist in managing patients better. I also help to educate health care assistants and carers in the basic administration of medications in the community setting and assist in teaching patients to self-administer certain forms of medication which requires effective techniques for example anticoagulation injections and respiratory inhalers.
I am part of the Trust operational group for the trust which is involved in completing Root Cause Analyses and Serious Untoward Incident investigations (SUIs) along with overseeing medicines management.
I accept instruction from both claimant and defendant solicitors and insurers. I have undergone formal training in the medico-legal process and am fully aware of my responsibilities under Civil procedure Rules (CPR) pertaining to expert witnesses.
