Intensive care unit, critical care unit
I qualified as a nurse in 1993 in the Philippines, where I worked in various roles whilst completing the academic requirements for the post graduate Master of Arts in Nursing. I specialised in medical surgical nursing and nursing management. This allowed me to take part in establishing a new nursing service office in a new hospital under the Department of Health in the Philippines. In this role I developed protocols and guidelines for every department under the nursing service. I also handled the staff rota/scheduling, training and accreditation at a mother-baby friendly hospital. In 2001, after moving to the UK, I worked in the emergency department. In 2006, I transferred to the Intensive Care Unit/Critical Care Unit (ITU/CCU) caring for critically ill patients, and I have remained working in this area, in various hospitals since.
In my current role, as a senior specialist nurse in ITU I am responsible for providing intensive nursing care to patients requiring complex multi body organ system support to maintain life towards recovery. My range of experience and skills include:
• Respiratory System support including oral endotracheal intubation, tracheostomy and laryngectomy care, mechanical ventilation using ventilators or non-invasive airway support.
• I am adept in following weaning protocols for gradual recovery and progression prior to removal of airway support.
• Cardiovascular system support, utilising multiple medications to maintain adequate blood pressure levels, maintain effective heart function and circulatory system such as those who have suffered stroke, and different forms of shock.
• Renal system support involving monitoring of kidney function commencing and monitoring the function and effectiveness of dialysis intervention for acute kidney injuries.
• Nutritional support for patients requiring nasogastric tube feeding, monitoring of blood glucose levels and administration of insulin preparations in order to maintain their ideal blood glucose levels.
• Neurological support to maintain adequate and safe levels of sedation in sedated patients.
• Post surgery patients who require close monitoring and extensive multi-organ system support requiring intensive nursing care. Some of these involve abdominal surgeries involving corrective surgeries, palliative surgeries and colostomy formation.
I am also responsible for organising and direction in the unit as part of my management role. I look after the junior staff in their learning by supervising and guiding them with the flow of the unit or work with them with a patient as part of their training. I also supervise students that are rotated in the unit as part of their nurse-training. I work with the nurse in charge and physicians in appraising them of the part of the unit that I cover as a senior staff with the concerns of other staff about their patients.
I accept instructions from both claimant and defendant solicitors and insurers. I have undergone formal training in the medico-legal process and am fully aware of my responsibilities under Civil Procedure Rules (CPR) pertaining to expert witnesses.
